Programs are required to maintain specific information for each clinical affiliate that participates in the clinical education of students.
Below is a form that contains the information that programs must maintain for each clinical affiliate. The program is required to maintain all information, preferably electronically, and update it annually. It will be reviewed by the AC-PE, at a minimum, as part of a comprehensive program evaluation/site visit.
